Underwear Fetishism<\/strong><\/p>\n\n\n\n

Underwear Fetishism is a paraphilic disorder\nthat can cause distress for the person who is affected by it and frequently\nleads to societal condemnation and rejection. Underwear Fetishism is\ncontradictive of normal sexual behavior and some view Underwear Fetishism as\nOCD-related sexual ideation. <\/p>\n\n\n\n

Fetishism has been recognized as a sexual variation for over 100 years\nand has very often been considered a disorder in all of its presenting forms\n(Martin, 2016). The German Psychiatrist Richard Von Krafft-Ebing identified\nparaphilias in 1886. The Buddhist Jathaka stories describe Fetishism in a young\nman named Kema\u201d (Jayatunge, 2014).<\/p>\n\n\n\n

Kafka (2003) defined paraphilias as disorders\ncharacterized by an alteration of sexual preference, volitional impairment, and\nan increase in sexual drive-oriented behaviors. Paraphilias are\npersistent, unconventional, and problematic sexual interests that exist on a\ncontinuum. Briken & Basdeskis-Jozsa (2010) highlight those individuals with\nparaphilias may experience negative factors such as low self-esteem, social\nanxiety, social skills impairment, depressive symptoms, and socially deviant\nbehavior.<\/p>\n\n\n\n

Fetishistic behavior can involve stealing or\nbuying undergarments and having an obsessive fixation with erotic fantasies.\nThey cannot become sexually aroused or reach orgasm without the object and feel unable to control their need\nfor the fetish. They are experiencing sexual arousal from undergarments\n(arousal from the smell of used women’s pantiesor\nvoyeuristic excitement viewing it).  Often, they prefer\nsolitary sexual activities associated with their fetishistic and sexualized\nfantasies.<\/p>\n\n\n\n

Fetishists become aroused by stealing the object, viewing the object, or\nmasturbating with the object. During\nmasturbation, the fetish object may be held, tasted, smelled, or used to stimulate\nthe genitals. They often feel guilty about their disorder and these fantasies, sexual urges\nand behaviors cause clinically significant distress or impairment in social,\noccupational, or other important areas of functioning in the person. Men appear\nto report greater interest in paraphilic activities than do women.<\/p>\n\n\n\n

Patients with fetishism often show a strong interest in non-genital\nbody parts. Female undergarments and shoes are among the most frequently\npreferred fetish objects. The person\nbecomes sexually aroused by wearing or touching the object. Most\nfetishists do not intend to cause harm to other people.<\/p>\n\n\n\n

Paraphilic disorder such as underwear\nFetishism is associated with acts that are deviations from socially accepted\nsexual behavior. Paraphilias are persistent and recurrent sexual interests,\nurges fantasies, or behaviors of marked intensity involving objects,\nactivities, or even situations that are atypical in nature.<\/p>\n\n\n\n

The International Classification of Mental\nDiseases (ICD-10th, World Health Organization 1992), paraphilias were\nclassified in the Sexual and Gender Identity Disorders. In the Diagnostic\nand Statistical Manual of Mental Disorders (DSM\u20135) the term\nparaphilia is defined as an intense and persistent sexual interest other than\nsexual interest in genital stimulation or preparatory fondling with\nphenotypically normal, physiologically mature, consenting human partners.\u201d<\/p>\n\n\n\n

In the DSM-5 these criteria should be\naddressed in the presence of three main aspects: first, the sexual arousal by\ndeviant sexual stimuli, second, the negative consequences for the individual or\nthe society and, finally, the fact that the person acts on his or her urges or\nthat the urges or fantasies cause significant distress, interpersonal\ndifficulty or impairment in functioning.<\/p>\n\n\n\n

Fetishism is described in both the\nmedical and legal literature. The term “fetishism” originates from\nthe Portuguese word feitico<\/em>, which means “obsessive\nfascination.” People with the\nfetishistic disorder may not be able to function sexually without their fetish.\nThe fetish may replace the typical sexual activity with a partner or may be\nintegrated into sexual activity with a willing partner. The fetishistic disorder\ntends to fluctuate in intensity and frequency of urges or behavior over the\ncourse of an individual\u2019s life. The existing literature indicates\nthat people with fetishistic disorder often\nreport a lack of self-esteem, a difficult childhood, and intrapsychic conflicts.<\/p>\n\n\n\n

The etiology of fetishism is not known. The\nexperts believe a combination of neurobiological, interpersonal, and cognitive\nprocesses all play a role. The level of an\nindividual’s sex drive is not consistently related to paraphiliac behavior. Paraphilias occur primarily in males\nwith an average onset between ages 8 and 12. They are a lifelong\ncondition. <\/p>\n\n\n\n

Some researchers began reporting a series of\nfindings linking paraphilias with brain structure and function. Two\nreported cases of fetishism have been associated with abnormalities in the\ntemporal lobe.  According to Wise (1985) temporal lobe epilepsy and temporal lobe tumor are\nlinked to the development of fetishism. Some experts believe that temporal lobe\ndysfunction and fetishism. They surmise that hippocampal agenesis\nis associated with hypersexuality and can be treated with carbamazepine.<\/p>\n\n\n\n

Biological explanations have included the\nnotion that perhaps paraphilia is related to hormones. However, research has\nfailed to show relationships between circulating hormonal levels and atypical\nsexual fantasies, urges, or behaviors<\/p>\n\n\n\n

In 1927 Freud argued that a fetish is a\nspecial form of penis substitute. Freud described fetishism as a defense\nagainst castration anxiety arising from the perception of the female\ngenitals.  According to Freud fetishism is a special kind of\nsplit within the subject, one that allows the male to sustain two incompatible\nassertions.<\/p>\n\n\n\n

Some theorists believe that fetishism develops\nfrom early childhood experiences, in which an object was associated with a\nparticularly powerful form of sexual arousal or gratification. A psychodynamic\nmodel of paraphilias may complement and add meaning to a neurodevelopmental\nmodel by viewing the paraphilic fantasies and behaviors as sexualized forms of\ndefence against underlying personality difficulties, anxieties, or conflicts,\nparticularly those concerning emotional intimacy<\/p>\n\n\n\n

Although the etiology of paraphilias is\nunknown, it is probably a learned behavior. The classical\nconditioning explanation for the origination of paraphilia seems to be the most\nwidely accepted theory.<\/p>\n\n\n\n

The main symptom of Fetishistic Disorder is a\nrecurrent and intense sexual arousal from either the use of nonliving objects\nor a highly specific focus on the non-genital body parts. Sexual fetishists\nfrequently need to be touching, smelling, or looking at their unique object or\nengaging in fantasy about it in order to function sexually alone or with a\npartner. Fetishistic Disorder is most often diagnosed through self-report of\nsymptoms.<\/p>\n\n\n\n

The treatment of Fetishistic Disorder is\nchallenging. Due to stigma the patients rarely seek professional treatment.\nTreatment is focused on decreasing the arousal to deviant sexual behavior.\nTreatment includes medication (antidepressants- Selective serotonin reuptake inhibitors and anti-anxiety medications) can also\nbe used in conjunction with psychotherapy.  Clinical success has been\nreported in behavioral therapy, social skills training, sex education, and\ncognitive behavioral therapy (CBT). The CBT is regularly followed by\npsychosocial education, family system treatment, multimodal treatment, and\nmultisystemic therapy. Some therapists use Sensatefocus therapy\nwhich was developed by Masters and Johnson in the 1960s to treat Fetishistic\nDisorder. Lijian Wu \u2013 a prominent EMDR therapist highlights using (Eye Movement\nDesensitization and Reprocessing) EMDR for fetish disorders. <\/p>\n\n\n\n
